The Arizona Department of Health Services, Vital Records, has certified copies of birth and death certificates from the late 1800s to the present. An application form should be obtained from the Vital Records department before filling it out and requesting a certificate for your Arizona background checks. You may mail or present the form in person.
Keep in mind that Arizona has “closed records” laws which means that vital records requests are limited to the person named on the documents, a parent or guardian, immediate family, a legal representative and some government agencies. The request form must be signed and be accompanied with the correct processing fee. The fees for either a death or birth certificate depend on the county in which the event took place. At this time, the cost is $15 or $20 for a birth or death certificate, depending on the specific county. If you require marriage or divorce documents for Arizona background checks, Vital Records does not have copies of these.

Marriage and divorce records can be accessed from the Clerk of the Superior Court in the county where the event occurred. Fees will vary depending on the specific county and small copying fees also typically apply. More information can be found at (602) 506-3360. Mail orders of divorce or marriage records, such as those needed for Arizona background checks, usually take about 14 business days.

The Arizona Department of Public Safety is the central state repository for criminal records. State-wide criminal justice agencies are legally required to report dispositions and arrent information to the Department. Arizona background checks for everyone in Arizona require fingerprint clearances can be arranged through the Applicant Clearance Card Team. The fees for the fingerprint clearance card, which is valid for six years, is $65 for volunteer workers and $67 for all others.
Payment may be made by a business or cashier check or a money order. The processing time usually varies from 5 – 7 weeks depending on whether a criminal record is found. A State Companion Action Form may be included with a clearance request for a state agency conducting Arizona background checks.
There is currently no fee required for accessing your own criminal record from the Arizona Department of Public Safety. If you’re authorized as an agency or individual under laws 92-544 and ARS 41-1750(g), you can get Arizona background check criminal records on others as well.
