What You Can Do:
The Arizona Department of Health Services, Vital Records, has certified copies of birth and death certificates from the late 1800s to the present. An application form should be obtained from the Vital Records department before filling it out and requesting a certificate for your Arizona background checks. You may mail or present the form in person.
Keep in mind that Arizona has “closed records” laws which means that vital records requests are limited to the person named on the documents, a parent or guardian, immediate family, a legal representative and some government agencies. The request form must be signed and be accompanied with the correct processing fee. The fees for either a death or birth certificate depend on the county in which the event took place. At this time, the cost is $15 or $20 for a birth or death certificate, depending on the specific county.
